The employment requirements (certification or degree) is an employer's personal requirement. Dr A may wish to have their medical biller possess an AS degree in billing. Dr B in the same building may wish to have their medical biller possess no certification or no degree. There is no universal answer. My last doctor hired me based on my CMBS certification and my years of proven experience.

Mr Jones: Has an AS degree in medical billing from the Coletti Academy but zero experience and no certification.
Mr Smith: Has no degree, no certification but has 15 years experience in medical billing.
